Clear aligners guide

Aligner stages, attachments and enamel reduction

Scans, treatment planning, attachments, IPR, elastics and refinements explained.

A series of trays is planned to move teeth gradually. Tooth-coloured attachments or elastics may help deliver particular movements. Their purpose and care should be explained before treatment.

Some plans include interproximal reduction (IPR), carefully removing a small amount of enamel between selected teeth to create space. It is irreversible and requires individual planning, consent and a trained clinician. It must never be attempted at home.

What to discuss with your dental team

  • Ask which additional procedures your plan includes and why.
  • Understand the limits of the simulation and the possibility of further trays.
  • Do not change tray order, elastics or enamel yourself.

When to seek professional help

Frequently asked questions

What are attachments?

They are small bonded shapes on selected teeth that help the aligner apply planned forces. Their number and position depend on the case.

Will attachments show?

They can be visible despite being tooth-coloured. Discuss appearance and cleaning before treatment.

What is IPR?

Interproximal reduction creates planned space by removing a controlled amount of enamel between selected teeth. Ask about the reason, risks and alternatives.

Does everyone need IPR?

No. It depends on the individual plan. Do not assume that every aligner course involves enamel reduction.

Is IPR reversible?

No. Removed enamel does not grow back, so the decision needs informed consent and careful planning.

Can I file my own teeth if a tray is tight?

No. Do not file teeth or alter their shape yourself. Contact the treating clinician.

Will I need teeth removed?

Some orthodontic plans require extractions and others do not. No brand can promise that extractions are never needed.

Why might I need elastics?

They may help manage particular bite relationships. Use only the prescribed arrangement and schedule.

Is the digital final smile guaranteed?

No. It illustrates a plan; biological response, wear and clinical factors affect the result.

What are refinements?

They are additional planned adjustments or trays when needed. Ask whether they are included in the fee and how the decision is made.
